They live in community organisations, voluntary groups, faith spaces and the everyday experience of people navigating a health system that was not always designed with them in mind.<\/li><li class=\"font-claude-response-body break-words whitespace-normal leading-[1.7]\">We believe that sustainable change does not come from the top down. It never has. Our first investment is in a High Intensity User service. Services focused on frailty, care coordination, and palliative and end of life care are now in development.
